 FOCUS GROUP CONSENT FORM: Guardian consent for child to Participation in our focus Group

What is the Research?

Your child has been sought to take part in a research project examining ecosystem studies using The Engineered Eco System, Eco-One by River Bottom Aquatics. The purpose of this study is to find out what advantages hands on, close observation studies have over traditional methods.

Why has your child been asked to take part?

Your child is between the grades of 5th and 8th. We would like them to take part in a discussion on how River Bottom Aquatics’ Eco-One system can best mirror elements of a natural environments identified in their educational curriculum.  We will talk about how hands on experimentation facilitates the learning process. 

Voluntary Participation

There is no monetary compensation for participants.  As a thank you, River Bottom Aquatics will provide pizza and beverages. After the discussion each qualifying participant will be entered into a drawing for one 10 gallon fish tank courtesy of River Bottom Aquatics. Participants may leave the group at any time for any reason, however you must be present to win.

Privacy

Your privacy will be protected. 

Your name will not be used in any reports or published advertisements. 

A video recorder will be used during the session only to document the discussion. It will not be used in any publicly displayed advertisements for River Bottom Aquatics.  

Videotape Permission

I have been told that the discussion will be recorded. I understand that my child’s name will not be publishes as a participant in this discussion. I understand that River Bottom Aquatics will not use video of my child in any public advertisement for their products.
